Through this website: your name, phone number, chosen hospital, and what you tell us about your dental concern, when you submit the appointment form.

At our hospitals: the clinical information needed to treat you — medical history, examination findings, radiographs, scans, photographs and treatment records.

To contact you about your enquiry, to provide dental treatment, to keep the clinical records we are obliged to keep, and — only with your separate, optional consent — to send you dental health information and offers.

Clinical photographs are part of your record. We publish a photograph of your treatment only where you have given written consent to publication, which is separate from your consent to treatment and which you can withdraw at any time. If you withdraw it, we take the image down.
